I understand that the following medications (paracetamol, Ibuprofen) are already provided at camp and will not pack any of them

I consent to receive emails from HolyGround until I request otherwise. Supplied information will be confidential and pursuant to the privacy act, and will only be used by the NEM team to effectively care for this camper and not used or any other purposes.

I consent for this youth to be transported to/ from camp by an NEM approved volunteer driver if a ride has been requested.

I agree for this youth to attend and fully participate in this camp, understanding that while some activities may entail an element of risk, steps will be taken to minimise any risks to this youth.

In the event of illness or injury, I consent for this youth to receive such treatment as is deemed appropriate by a delegated camp first-aid staff. E.g: wound care, paracetamol.

If further treatment is required and the emergency contact is unable to be contacted, I consent to this youth receiving required treatment until the emergency contact can be contacted.

By checking this I DO give permission for any images of this youth obtained during camp to be used in the promotion of camp through the HolyGround website and social media.

In confirming, I agree to help this camp run smoothly. I understand I need to co-operate, ensuring that my words, attitude and behaviour reflect the love of Christ. In registering I am expressing my desire to see Jesus honoured in all areas of camp life, and my willingness to abide by camp rules. I accept the authority of those in positions of leadership. I understand that I may be sent home if I am unable to follow camp rules or am negatively impacting the experience of others.
